When booking an appointment, we require that you use our third-party booking software provider (“Jane”). We use this clinic management platform to collect personal information from our patients and create patient records.

Such records include a patient’s name, address, health insurance information, billing data, medical charts, appointment details, and additional patient details. If you are a patient, this data is collected during clinic visits and the creation of an account via Jane’s third-party online booking platform. How We Get Your Consent

Any links to external websites found on our Site are not affiliated with us, and we are not responsible for the privacy practices of those external organizations. Additionally, we are not responsible for the privacy practices of websites that may link to our organization.

Payment

We only retain payment information that you have explicitly agreed to provide and authorize for use, either at checkout or through the pay balance and Jane account. We handle your payments with care and confidentiality in accordance with all applicable privacy laws and regulations. All payments are processed through Jane. When you make a payment through Jane, you may do so via Stripe or Square, which are processed with a connection to credit cards such as VISA or Mastercard.

If necessary, we may update our Policy. If there are changes, we will provide notice on the Site.
